1095B Health Coverage Form for Terminated Employees

Previously, IRS guidelines required the City of Port Huron to send a 1095B Health Coverage form to terminated employees with a balance in their HCSP accounts.  The IRS has changed this guideline and forms will now be available upon request only.  For a copy, please send a request to
